It is important that patients receive the best in information here because of immunization records as well as any concerns when the child was born up until this time. This information should always be passed from doctor to doctor and in most cases the doctors will ask you to sign off on this transfer of information.

There are some strides being made to help this though. For example, much of the medical-records are now being put on computers in databases that doctors share. Also, you will find that many of the doctors are using electronic means for recognizing patients and conditions. It is your responsibility, as the patient, to make sure that medical-records are passed from place to place throughout your care.
