Root Canal Swelling

Swelling after root canal is one of the possible complications of root canal treatment. Given below are the various causes and treatment options for dealing with this complication.
A swelling that occurs in the innermost section of the root, that is, in the root canal, is known as a root canal swelling. This situation normally occurs when there is accumulation of pus in the root canal. This leads to severe pain, however, the term 'root canal swelling' is a misnomer, as the canal itself does not distend and actually swell in size, but the adjacent tissues, like gums, swell up. However, there is build up of severe pressure in the canal, which leads to pain in the tooth and swelling around it. This kind of swelling can be seen before undergoing root canal treatment as well, as result of acute or chronic pulpitis, which leads to build up of pus and pressure in the root canal. However, more often than not, when speaking of swelling, one usually refers to swelling after root canal treatment, which is one of the most common root canal complications. Given below are the various possible causes and treatment options to deal with the swelling.

Causes

Faulty Root Canal Procedure
There are many things that can go wrong while doing a root canal procedure. After cleaning the root canal, there is placement of a sterile material in the canal which is known as gutta percha. Sometimes, the gutta percha placement may go beyond the tip of the root, and may enter the periapical bone, which could lead to a swelling. On the other hand, if sufficient number of gutta percha sticks are not placed in the root canal, then the empty space may act as a source of infection, which could lead to swelling and pain. In a worst case scenario, a part of the surgical instrument (files) might break and get left behind in the root canal. This acts as a nidus for root canal infection.

Re-infection
Sometimes, after a root canal procedure, if all pulp tissues are not removed, there may be a re-infection of the canal, which could lead to swelling and pain in the region of the treated tooth. Furthermore, sometimes there may be a slight infection in the periapical bone, which may not be visible in the x-ray, which can also be a source of infection, as there may already be active proliferation of microorganisms in this region. These microorganisms may further progress towards the root canal, causing root canal pain after procedure along with the swelling.

Inability to Follow Post Treatment Instructions
Often it is seen that patients, after completion of their root canal procedure, never return to the dentist even if the dentist insists on a follow-up appointment. This follow-up appointment is only asked for to ensure that there are no further untoward root canal problems that the patient may have to face. Also, if the patient is asked to not eat from the side of the root canal treated tooth, it is strongly advised that he follow this instruction, as immediately after root canal treatment the tooth is often susceptible to fracturing under the impact of masticatory stresses. Sometimes, if there was a lot of loss of tooth structure, then the root canal treated tooth may require a crown. If the patient does not return to get this tooth capped, then there are high chances of the tooth eventually fracturing and the canal opening being exposed, which is an open invitation to bacteria and other microorganisms.

Treatment

Thus, when it comes to treating post root canal swelling, it is imperative that first the cause of the swelling is diagnosed. This can easily be diagnosed with the help of an oral examination of the tooth and a clear x-ray. Thus, if there is a broken instrument in the root canal, it is important to try to remove it. However, if there is an infection in the root canal, then the swelling can recede with the help of some antibiotics. However, if there are some other problems with the tooth, like if the tooth becomes mobile, or completely fractures, then it may require extraction.

Thus, root canal swelling is normally an untoward and highly avoidable aftermath of root canal procedure. Thus, it is highly encouraged that the dentist perform the procedure thoroughly and the patient follows the post treatment instructions religiously, to prevent any such complications.
