The Scot, celebrating his 20th birthday today, was 5-1 up in the first set when he was left wringing his right wrist in intense pain after returning a powerful shot from the Italian.
The British No1 had looked in far better form than last week, when he exited the Rome Masters at the first hurdle, but he was forced to quit the match two points after receiving treatment from the tournament physio.
Murray now has 13 days to recover before the French Open begins at Roland Garros.
